MENTORS+ YOUTH REFERRAL CRITERIA
For an adolescent to be considered for a Mentors Plus match, he or she must be referred to Chaffee County Mentors Plus by a youth service professional associated with, but not limited to: Chaffee County Health & Human Services, Chaffee County public schools, local youth organizations and juvenile detention efforts.
Specific Criteria:
- Youth must be between 14 and 18 years of age
- Youth is experiencing challenges in the home, school, socially and/or economically
- Youth is involved or at-risk of involvement in the juvenile justice system
- Youth/family is involved in services provided by HB1451/Wraparound, SB94, DHS/FYI, or other similar services
- Person referring can identify specific goals for the youth
- Both youth and parent/guardian desire the service
- Youth has full support and commitment from parent/guardian
Youth Service Referral Professional Referral Steps:
- Contact and speak with parent/guardian about Mentors Plus program
- If parent/guardian would like more information, call Mentors Plus with their contact information
- If parent/guardian agrees to program and youth is interested, fill out Referral Form and mail, fax or email to Mentors Plus
- Mentors Plus will then contact parent and youth and take appropriate next steps for youth’s involvement.
- Person who referred the youth will be contacted by Mentors Plus to determine your role in supporting the match.
