TRADITIONAL MENTORING - YOUTH REFERRAL CRITERIA
For a young person to be considered for a mentor match, he or she must be referred to Chaffee County Mentors by a youth service professional associated with, but not limited to: Chaffee County Health & Human Services, Chaffee County public schools, and local youth organizations.
Specific Criteria:
- Youth must be between 7 and 15 years of age
- Youth is experiencing challenges in the home, school, socially and/or economically
- Youth is seen or identified as being “at-risk” of involvement in the juvenile justice system, but is not yet involved
- Both youth and parent/guardian desire the service
- Youth has full support and commitment from parent/guardian
Youth Service Referral Professional Referral Steps:
- Contact and speak with parent/guardian about Chaffee County Mentor program opportunities for their child. Ideas of what to say: Introducing Parents to CCM
- If parent/guardian would like more information, call Chaffee County Mentors with their contact information
- If parent/guardian agrees to program and youth is interested, fill out Referral Form and mail, fax or email to Chaffee County Mentors
- Chaffee County Mentors will then contact parent and youth and take appropriate next steps for child’s involvement.
